Please note: This participant information sheet and waiver only needs to be completed once per child and the information will be kept on file for their future drop-ins. There will be no Youth Open Gym Time on the following dates: September 16th, 2022 September 30th, 2022 November 11th, 2022 March 17th, 2023 April 7th, 2023
